What to Bring

1. Completed Application

1. Download the DS-11 Passport Application here.
2. Complete the DS-11 Passport Application but DO NOT SIGN.

2. Proof of U.S. Citizenship

Certified U.S. birth certificate with:

• Official seal
• Full name, date of birth, place of birth
• Parent(s)’ names
• Filing date within one year of birth
• Registrar signature
  OR
• Full-validity, undamaged U.S. passport
  • 10 years (adults)

  • 5 years (minors)

     

• Consular Report of Birth Abroad or other U.S. Department of State birth document
• Certificate of Naturalization or Citizenship.

3. Valid Photo ID

Most commonly accepted:

• Valid driver’s license
• Enhanced driver’s license with photo

YOU MUST PRESENT THE PHYSICAL CARD. 

America's 250th: State Department Commemorative Passport

The Department of State created a special U.S. passport ("Patriot Passport") to commemorate the historic occasion of America’s 250th anniversary. This special passport has custom artwork and enhanced images on the front, back, and inside covers. It has all the same advanced security features that make the U.S. passport the most secure travel and identity document in the world. The Department is now offering Americans across the country the opportunity to apply for one of these 250,000 special and historic passports.

5. Fees (Credit and Debit Card Payment Suggested for this Event)

• Adults (16+):
  • $130 passport book ($30 card optional)
  • $35 acceptance fee
• Minors (Under 16):
  • $100 passport book ($15 card optional)
  • $35 acceptance fee
