Congresswoman Kelly Announces Five Community Health Center Grants
Aunt Martha's Health & Wellness, along with four other community health centers in the South Suburbs and on Chicago's South Side, have been awarded a total of $273,000 in federal grants from the U.S. Department of Health & Human Services.
"Community health centers are a powerful way to tackle health disparities and deliver quality healthcare directly in our communities," Congresswoman Kelly said. "Our local community health centers actively reduce barriers to accessing care by bringing care into the community. The ACA made major investments in community health care and community health centers, and it's vital that we continue to expand that model."
Aunt Martha's will use its $136,000 grant to expand their telehealth capabilities to include remote monitoring of patients suffering from chronic disease. It will also continue to refine its integrated care model, which coordinates general and behavioral healthcare to deliver better results for patients.

"This award also reflects Congresswoman Kelly's hard work and advocacy for health centers in the 2nd District, across Illinois and across the country."
Aunt Martha's has locations across Chicagoland, including Harvey, Chicago Heights, Park Forest, Hazel Crest, Kankakee, and more.
Other grant recipients include:
- Chicago Family Health Center, Inc., - $56,000,
- Family Christian Health Center, Inc. - $36,000,
- Christian Community Health Center - $40,000 and
- TCA Health, $5,000.
