What to Bring

1. Completed DS-11 Passport Application (unsigned)
2. Proof of U.S. Citizenship (bring ONE):
   • Certified U.S. birth certificate
   • Full-validity U.S. passport (10 years adult / 5 years minor)
   • Consular Report of Birth Abroad
   • Certificate of Naturalization or Citizenship
3. Valid Photo ID (driver’s license or enhanced driver’s license)
4. Passport Photo—must meet official requirements: Photo Guidelines
5. Fees (separate payments required):
   • Adults (16+): $130 passport book ($30 card) + $35 acceptance fee + $60 mandatory expedite fee
   • Minors (<16): $100 passport book ($15 card) + $35 acceptance fee + $60 mandatory expedite fee
